The American medical system can be quite different from what you are accustomed to at home, so we can assist you in obtaining the care you need.
Medical Record Checklist
All international students are required to submit their health forms (PDF) to Student Health Services prior to their arrival at orientation. Fall students by August 15 and spring students by December 28.
Please carefully review the last page, Tuberculosis Risk Assessment. If you answer “yes” to any of the three questions, you will need to have a tuberculosis test done in the United States, not in your country of origin. If you require a TB test, it will be performed the first day of orientation. The cost of the test will be billed to your student health insurance.
All of the listed vaccines are required by law. Please bring proof of all of the vaccines that you have previously received. The University Health Insurance Plan will cover the costs of vaccines that you still need. Your failure to submit these records will result in your inability to register for classes.

Transfer Students
Students transferring from other U.S. institutions should be aware that vaccination requirements are not uniform among all U.S. colleges and universities. As such, even if you have met the vaccination requirements at your current campus, you might not meet the health requirements for Fitchburg State.
Please examine Fitchburg State's requirements carefully to determine whether you need additional immunizations. The easiest way to provide Fitchburg State with your medical records from your past institution is to download the Authorization for Release of Medical Records (MS Word) and fax it to your past institution. They will fax Fitchburg State your medical records. Be sure to initiate this process in time for your records to reach our Health Services office prior to Orientation.
Health Insurance
All international students taking classes on campus are required to enroll in the Fitchburg State University student health insurance plan and will be responsible for the fee. The policy is effective from August 1 to July 31.

Failure to comply with these requirements may prohibit you from registering for future classes. If an international student has comparable coverage they may request a waiver by completing the online Health Insurance Waiver Request form and submitting the required documents. The required documents are: Summary of Benefits, policy dates and copy of insurance ID card.
Students with coverage from insurance carriers outside the United States or coverage by foreign National Health Insurance programs may not waive participation in the University’s plan. These plans are not deemed to be comparable to coverage under a QSHIP program. This is a change in the QSHIP regulations that has been mandated by the Division of Health Care Finance and Policy and all institutions are required to comply with this change.
